Coordinated Care of Washington (CCW) administers the Apple Health Core Connections program. This program will serve children and youth currently in foster care and adoption support, and young adults aged 18 through 25 who are alumni of the foster care system. This presentation will be an overview of the Apple Health Core Connections program and answer questions in relation to the program objectives.
Knows how to advocate and obtain assessments, treatment, and services for children in care for health, mental health, developmental delays, and other issues.
Able to attempt to access resources, providers and funding available to meet the needs of children; able to redirect efforts with the child welfare team for shared solutions.
Utilizes a wide array of skills, knowledge, and resources to seek medical and/or therapeutic services appropriately to treat serious or chronic illnesses or medical conditions.
